Continuing Review / Renewal

Due to the COVID-19 pandemic, some of the usual REB processes described below have been temporarily changed. For the most up to date information on REB operations, please see the REB COVID-19 Information and Updates page.

Studies are granted Research Ethics Board (REB) approval for a maximum of one year and must be renewed in order to continue. To extend REB approval for a further one year period, a continuing review request must be submitted to the research ethics office, located at 250 Yonge St., 6th floor.  It is the responsibility of the principal investigator to ensure that a complete continuing review form is submitted to the REB prior to the study expiration date.

Please follow these instructions to request a continuing review or renewal for a study:

Submit completed continuing review form

  1. Complete a continuing review form, with an original signature of the Unity Health Lead Applicant.
  2. Provide two (2) hard copies of the completed form to the research ethics office, located at 250 Yonge St., 6th floor. Incomplete forms will not be accepted.
  3. One copy of the form will be date stamped and given back to you for your records.

Important:

  • Submissions that are incomplete or filled out using an out-of-date form will not be accepted.
  • Continuing review forms should be submitted prior to the study expiry date, but no earlier than 60 days before expiry.
  • The continuing review form cannot be used to submit documents to the REB for acknowledgement or approval. Amendments, safety information, and changes to investigators/study personnel should be submitted separately.
  • Failure to submit a continuing review form by the study expiry date will result in study closure.

Review of the continuing review form by the Research Ethics Board

  1. During review of the continuing review application, the Research Ethics Coordinator will contact you for any clarifications or outstanding training certificates that are needed prior to renewing the study.
  2. The contact person for the continuing review will typically be the Research Ethics Coordinator that worked on the original application. If you don’t have a Research Ethics Coordinator, please email PatelD@smh.ca.
  3. There are sometimes cases where a continuing review application requires review by the full board. If this occurs, the Research Ethics Coordinator will let you know and inform you of the next steps.

 

Approval of the continuing review application

  1. Once the review is complete and all concerns have been addressed, the continuing review application will be approved. 
  2. You will be notified when the approval letter is available for pick up in the research ethics office.
