Research Staff Change

Please follow these instructions to change all study staff:

Submit completed staff change package

  1. Complete the staff change form, with an original signature of the Unity Health Lead Applicant and any added or removed Co-Investigators. 
    • Include amended versions all approved documents that are impacted by the change.  One clean and one tracked changes version of all amended documents must be provided.
  2. Provide two (2) hard copies of the staff change package to the research ethics office, located at 250 Yonge St., 6th floor.

Review of staff change package by the Research Ethics Board

  1. Staff changes are reviewed on a first-come, first-served basis. 
  2. The contact person for the staff change will typically be the Research Ethics Coordinator that worked on the original application. If you don’t have a Research Ethics Coordinator, please email
  3. During review of the application the Research Ethics Coordinator will contact you with any clarifications that are needed prior to approving the staff change.

Approval of the staff change application

  1. Once the review is complete and all concerns have been addressed, your staff change application will be approved or noted to file.
  2. If an approval letter is issued, you will be notified when the approval letter is available for pick up in the research ethics office.
